1. Log in to the IBITA website as a member and follow these steps. The images below are not linked as they are specific to your login.
2. Go to and check your profile is accurate
3. Go to and make any corrections if needed
4. Go to and complete the survey
- PTPF Upload: Basic Course and Advanced Course Instructor-Candidates shall be required to upload your updated Personal Training Profile Form (PTPF) with your SIS submission. Be sure to update your PTPF and have it ready for upload before you start your SIS survey. ONE file only can be uploaded, if required please combine files into one PDF.
- Basic Course Instructor Candidates please ensure the file name reads:

5. A copy of your responses will be emailed to you
6. If you wish to change your responses please submit a new SIS to replace prior submissions.

Member Duties to Submit a Statement of Instructor and Instructor Candidate Status (SIS)

3. A member has the duty to submit a Statement of Instructor Status each year, stating he/she has fulfilled the requirements for maintaining membership and instructor status and has fulfilled the requirements to have voting rights at the AGM.
4. An Advanced Course Instructor-Candidate has the additional duty to submit the Statement of Instructor-Candidate Status section of the Statement of Instructor Status, giving information on the progress in his/her training process.
5. An Associate Member (Instructor-Candidate) has the duty to submit a Statement of Instructor- Candidate Status giving information on the progress in his/her training process.
Procedural Manual, 2.4 Statements of Instructor Status and Instructor Candidate Status (SIS)
- All members are required to complete the SIS.
- The SIS survey must be submitted after 1 May and before 1 July (Byelaws Art. VI – Members’ rights and duties – Section II Duties of members – Clause 3).
- Exemptions must be applied for before July 1, the SIS submission deadline.
